Check-In Forms
Would you like to make your check-in with us as relaxed and time-saving as possible?
This gives you the chance to prepare some of the documents in the comfort of your own home. If you bring them with you already completed, things will go much more quickly when you arrive, and we can focus straight away on your stay and your dives.
You can print out the following check-in documents in advance and bring them with you, already completed:
Bitte achtet außerdem darauf, eurer Brevet bzw. euren Taucherpass mitzubringen, da wir diesen für die Anmeldung und Dokumentation eurer Tauchaktivitäten benötigen. Die geltenden Tauchbestimmungen für Malta findet ihr bei der PDSA
You must always submit a completed health declaration form. If your health declaration contains information that requires a medical certificate, please also bring a valid diving medical certificate with you. This ensures that you can enter the water safely and without any issues.
If you’re unsure beforehand which documents are required in your case, we’ll be happy to help. We’ve summarised some frequently asked questions in the next section.
What should I do with the forms if I don’t have a printer?
You can fill in the forms directly on your computer or tablet. You can then simply email them back to us. There is no need to print them out.
By when should I complete and send the documents?
It’s best to fill in the documents a few days before your arrival and then bring them with you or email them back in advance. That way, we can check everything in advance and speed up your check-in on arrival.
Do I need to check the health declaration carefully beforehand?
Yes, please read the health declaration carefully. Depending on the information provided, a diving medical certificate may be required. If you do not have one, you must make sure to allow enough time before your holiday to book an appointment with your sports doctor.
Can I fill in the forms when I get there?
Yes, that’s possible. You can also fill in or complete all the documents on the spot. However, check-in will then take a little longer than if you’d prepared them in advance.
Do I need to bring my diving certificate with me when I check in?
Yes, your diving certificate is required for check-in. It serves as official proof of your diving training and is required as evidence.
Is a digital certificate sufficient?
Yes, in most cases a digital licence (e.g. on a smartphone or tablet) is perfectly adequate. It is important that all the relevant information is clearly legible.
What happens if I don't have my diving certificate with me?
You cannot go diving without valid proof of your diving qualification. If you can’t find your certificate, please contact us in advance so that we can find a solution.
Will my certificate be checked on site?
Yes, we’ll check your certification at check-in to ensure you’re diving in line with your training and experience.
Do I need to bring my logbook with me when I go diving?
No, your logbook isn’t compulsory. However, it is helpful for better assessing your diving experience to date.
Is a digital logbook sufficient?

What happens if I don’t have a logbook?
No problem – you can still dive with us even without a logbook. In that case, we’ll rely on your certification and the personal chat we have with you at check-in.
